1) As stated in the Ohio Revised Code, OVRS may not place a person in a position that involves providing services directly to individuals with a developmental disability, if the person has been convicted of or pleaded guilty to any of the following, or a felony that bears direct and substantial relationship to the duties of the position being filled.  Rehabilitation consideration can be given as defined by the Ohio Revised Code.  Internal policies within OVRS regulate employment regarding other offenses.

4) I agree that after employment, I will notify OVRS, within 14 days, of any formal charges that would appear on a criminal record.  This includes convictions or guilty pleas to any of the above disqualifying offenses.
